Allow multiple Approval templates for one approval type

For Service Request approvals depending on which service they are for different fields might be of interest to the end users, therefore we would like to be able to define multiple templates that we can assign to specific catalog items. 

We want to declutter the approval notifications to show only the relevant information to the users.
