Analytics Miscalculating "Unsuccessful" Knowledge Base Metrics

In Analytics, we review the Unsuccessful Knowledge plugin to locate gaps. We noticed that when a form is served, we received an “unsuccessful” for knowledge. Is anyone else seeing this? What are you doing to remedy this issue?
